マイナポータルの「健康保険証」または日立健保から案内された「資格情報のお知らせ」、
「資格確認書」を確認し、２つの記載事項を下の枠に入力してください。
Check the ‘Health Insurance Certificate’ on Mynaportal or the ‘Eligibility Verification Certificate’,
the 'Notice of Eligibility Information' provided by Hitachi Health Insurance Society,
and enter the two items in the box below.
