マイナポータルの「健康保険証」または日立健保から案内された「資格情報のお知らせ」、
「資格確認書」を確認し、２つの記載事項を下の枠に入力してください。
Check the ‘Health Insurance Card’ on Mynaportal or the ‘Eligibility verification certificate’,
the 'Notice of e ligibility information' provided by hitachi health insurance society,
and enter the two items in the box below.
